Complete your health assessment to earn a $250 reward.
Then look for the "Reduce My Premium" activity and complete it to earn another $50 reward.
Complete up to four self-guided health and wellness programs and earn a reward of $50 each, these are $200!
You will see the amount of the reward on your next month's bill. We will apply the reward to your premium, until all is used.

Log into your member account at floridablue.com. Click on Health and Wellbeing, then on Better You Strides.
Read and accept the Terms of Service.
Choose your communication preferences.
Complete your Personal Health Assessment.
Download the AlwaysOn app, available through the Apple App Store or Google Play. Click New User. To view the AlwaysOn mobile app in Spanish, please change the language setting on your smartphone to Spanish before downloading the app.
Enter your name, date of birth, and zip code. Click Verify.
Complete the authentication steps. You will be asked to enter your Group number, which you will find in your member ID. If you don’t have your member ID card, simply log into your Florida Blue account to download a copy of your card or access it from the Florida Blue app.
Enter a username, password, mobile phone number, and email address. Accept the terms of service, then create your PIN.
